Registration takes place at the Welcome Center. You will need to personally appear to finish registering your child and provide required documentation. You will need to bring proof of age, proof of residency, and proof of immunizations to the Welcome Center. You can speed up the process greatly by filling out the forms online prior to coming in. When filling out the forms online, we recomend you use the Firefox, Chrome, or Safari browser.

Students age 5 through 20, who have not already earned a high school diploma, satisfied graduation requirements for a diploma, or completed high school educational requirements in a foreign country are eligible to attend MCSD if the student resides within the school district boundaries on a full time basis with their custodial parent or court appointed legal guardian. Eligible students, age 18 or older, who are living on their own, may be eligible to enroll in school. Those students shall contact the Welcome Center for further assistance.
In limited and specific circumstances a kindergarten student may be eligible for an early entrance or a delayed entrance into kindergarten. Please see the Student Services Manual, Section I, and Section IV for more information.
Middletown City Schools has six elementary schools which students are assigned to by address. The schools are Amanda, Creekview, Mayfield, Miller Ridge, Rosa Parks, and Wildwood. You may enter your house number and street below to see which elementary school your children are assigned to. Keep in mind that because of overcrowding in some schools, the welcome center may place your children in an alternate overflow school. If this happens, your children will be provided with transportation. If you have questions, you may call the Welcome Center at 513-420-4561.
Only the biological parent, custodial parent, or court appointed legal guardian may enroll a child in school. A step parent, other relative, or non-relative shall not enroll a child. If you are the non-custodial parent, you must have a notarized statement from the custodial parent granting permission for the child to live with you, as well as, grant permission for you to enroll the child in school and make educational/medical decisions for the child. If you are not the biological/custodial parent or court appointed legal guardian, contact the Welcome Center to inquire about enrolling a student.
All students must have legal proof of age, proof of residency, and proof of immunizations in order to enroll in school. Proof of Age- One of the following documents, listed in order of preference, must be provided. The original document must be presented and a copy will be made.
